Medications at School
If you are in need of bringing medication to the office, we encourage you to complete the forms prior to your arrival. We hope that will expedite the amount of time it will take you to drop off medication.
We want to remind you that it is Oregon State Law that parents must transport all medication to school, whether prescription or non-prescription. In addition, it is Oregon State Law that we must have parent consent and instructions in writing. TTSD has a form that meets this need. Medication Administration Record and Parent Consent.
In addition, there might be times that it is appropriate for a student to self-administer medication. If that is the case, the Principal, District Nurse, Parent and Student must all sign the Permission to Self Medicate.
