Tigard-Tualatin School District kitchens process all major allergens. Due to supply chain constraints, we are also receiving substitutions from our vendors without notice.

If you have a food allergy, please connect with your school nurse.

1. Parent/guardian, please complete the Severe/Anaphylactic allergy questionnaire for the student.  
2. Any student with food allergies who requires a SEPARATE MEAL different than was offered needs a physician to fill out this form .
